Founded in 1786, San Fernando was named after Saint Ferdinand III of Castile, Spain. In 1870, it was made the capital of La Union.
Paulino Alviar was the appointed mayor of the city from 1895 to 1898. He was succeeded by Blas Tadiar who ruled the city from 1899 to 1901.
On March 20, 1998, it was converted into a component city with Mary Jane C. Ortega as the mayor of the newly created city.

The City of San Fernando is a regional center of commerce. It is home to the former United States Wallace Airbase which has been turned into the Poro Point Economic Freeport Zone. This economic zone is envisioned to be an investment and tourism hub offering generous incentives for locators.
